Community Paramedicine

Our community paramedicine program was created to address any unmet health needs of our community. As needs rise, Cumberland Emergency Medical Services (CEMS) is presented a unique opportunity to be partners in the larger health ecosystem, a system where traditional health care providers are linked with community-based organizations in a coordinated system of care. CEMS practitioners have traditionally functioned as a mobile healthcare unit and have become the logical resource for providing healthcare to the community as an extension of the primary care and public health network.

In the simplest definition, community paramedicine is the provision of healthcare using patient-centered, mobile resources in the out-of-hospital environment. It may include, but is not limited to, services such as providing telephone advice to 9-1-1 callers instead of resource dispatch; providing community paramedicine care, chronic disease management, preventive care or post-discharge follow-up visits, disease program referrals, home risk assessments; or transport or referral to a broad spectrum of appropriate care, not limited to emergency departments.
